简要规则描述： 本次维修工程项目范围为：本次大修工程项目范围6幢主体建筑和其他附属用房。主要维修内容：对房屋建筑、消防设施、供水供电、雨污水等进行整修。本工程包工、包料、包工期，包质量、包安全文明施工、包验收。具体技术要求详见图纸和工程量清单。

Brief Specification Description: The scope of this maintenance project includes 6 main buildings and other ancillary buildings. Main maintenance tasks: renovating buildings, fire-fighting facilities, water and power supply, rainwater and sewage, etc. This project includes labor, materials, construction period, quality, safe and civilized construction, and acceptance. Please refer to the drawings and bill of quantities for specific technical requirements.

二、申请人的资格要求

2. Qualification Requirements for Suppliers

(a)满足《中华人民共和国政府采购法》第二十二条规定；

(a)Meet the provisions of Article 22 of the "Government Procurement Law of the People's Republic of China";

(b)落实政府采购政策需满足的资格要求： 本项目专门面向中小企业采购，中小企业应当按照规定提供《中小企业声明函》。

(b)Qualification requirements to be met to implement government procurement policies: This project is specifically aimed at small and medium-sized enterprises for procurement, and small and medium-sized enterprises should provide a "Small and Medium sized Enterprise Declaration Letter" in accordance with regulations.

(c)本项目的特定资格要求： 3、未被“信用中国”（www.creditchina.gov.cn）、等列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单；
4、施工资质要求：具备国家住建部颁发的建筑工程施工总承包三级以上（含三级）资质及建筑装修装饰工程专业承包二级以上（含二级）资质，且具备有效的安全生产许可证；
5、项目负责人资质要求：拟派的项目负责人具有建筑工程二级或以上注册建造师执业资格，并具备有效的安全生产考核证书（B证），且未在其他在建项目担任项目负责人（注册建造师基本情况及是否有本市的在建项目以供应商提供的开标前两个工作日上海市建设市场管理信息平台采集的数据内容形成的《项目负责人基本情况表》为准, 项目负责人在履行合同过程中发生变更的，如变更后时间未满半年，不得参与本标段投标。）；
6、法人的分支机构以自己的名义参与竞争时，应提供依法登记的相关证明材料和由法人出具的对该参与竞争活动承担全部直接责任的承诺书；
7、法定代表人为同一人或者存在直接控股、管理关系的不同供应商，不得参加同一合同项下的采购活动；
8、本项目不接受联合体响应。

(c)Specific qualification requirements for this program: Construction qualification requirements: Possess the third level or above (including third level) qualification for general contracting of construction projects and the second level or above (including second level) qualification for professional contracting of building decoration projects issued by the Ministry of Housing and Urban Rural Development, and have a valid safety production license;

(i)符合《中华人民共和国政府采购法》第二十二条的规定；

(i)Comply with the provisions of Article 22 of the"Government Procurement Law of the People's Republic of China";

(ii)未被“信用中国”（www.creditchina.gov.cn）、列入失信被执行人、重大税收违法案件当事人名单、政府采购严重违法失信行为记录名单；

(ii)Not included in the list of dishonest persons subject to enforcement, the list of parties to major tax violation cases, or the list of records of serious illegal and dishonestacts in government procurement by "Credit China" (www.creditchina.gov.cn), China Government Procurement Network ;
